Health Benefits

At Concept Rehab, Inc., we provide you with an array of benefit choices. We know that one size doesn't fit all, so we've designed plans to give you flexibility and choice.

Concept Rehab, Inc. offers a range of health care choices including: Medical, Dental, and Vision plans as well as a Health Savings Account and Health Care and Dependent Care Spending Accounts.

Employees may choose from one of our Anthem Blue Cross Blue Shield plans: Health Savings or a Preferred Provider Organization (PPO) Plan. Coverage is effective the first day of the month following date of hire and cost of the plan are based on your coverage level. The company provides each employee who elects a medical plan with Basic Life Insurance.

Employees have an option of purchasing additional coverage through our Voluntary Insurance Program which features Voluntary Life Insurance, Short Term and Long Term Disability protection when unexpected accidents and serious health conditions occur.

401(k)

Concept Rehab, Inc. offers a 401(k) Savings Plan with a company provided match. Employees may choose from pre-selected global portfolios or they may choose from numerous mutual funds to make up their own investment mix.

Vacation/Personal Time/Holidays

Concept Rehab, Inc. has six paid holidays each calendar year and an annual vacation plan based on years of experience. Additional paid personal time is given each year.
