As Value-Based Purchasing programs evolve and reimbursement models become more complex, nursing home leaders are asked to manage quality, compliance, and financial outcomes simultaneously. Without alignment, opportunities are often lost.

A New Approach: Integration That Drives Outcomes

A not-for-profit Continuing Care Retirement Community (CCRC) recognized these challenges and partnered with Concept Rehab and Engage Consulting to implement the Integrate Advantage model. This approach connected therapy services and MDS consulting into a cohesive, collaborative strategy focused on improving quality outcomes, documentation accuracy, and financial performance.


By aligning teams around shared goals, the organization was able to break down silos, ensure MDS accuracy reflected resident needs, and proactively manage performance across QRP, PDPM, and state incentive programs. Therapy became a strategic driver of functional outcomes, quality performance, and reimbursement protection.


The Results: Measurable Impact

The results were clear and significant. Financial gains included:

  • $16.81 increase per Medicaid resident day

  • Improvement in Quality Incentive Program (QIP) capture from 31.25 to 46 points


Alignment between therapy and MDS also supported more accurate reimbursement.

Clinically, residents experienced meaningful improvements.

  • Discharge Function Scores increased by 32%

  • Self-Care at expected discharge levels rose by 23%

  • Mobility at expected discharge levels improved by 44%


The organization also saw a +1 Star improvement in Quality Rating and earned 90 additional points toward the overall Five-Star rating. Audit readiness and compliance confidence strengthened across the board.
