About the event
Taking the stage at this year’s PAC Symposium is nationally recognized speaker, Monica Westhead. Attendees will learn the impact of avoidable hospital days and strategies for acute and post-acute providers to work together to manage patients before, during, and after care transitions. The presentation will address:
- Methods for working with sending and receiving facilities to deliver efficient, high-quality care
- Tactics to address recurring gaps in routine care during transitions and following an episode’s end
- Strategies to mitigate the impacts of five common causes of delayed transitions
In addition, a panel of Concept Rehab experts will discuss the state of our post-acute care industry as we prepare for the sweeping changes of PDPM come October.
